DESCRIPTION OF REQUESTED SERVICES: This individual is looking for an HPC provider to work with in his home. The family is looking to start with 4 hours of services per week, with the individual’s mother/guardian providing training. He needs physical to total assistance to complete all hygiene tasks, meal plan and prep and navigate across surfaces. He uses a walker in his home and a manual wheelchair for any long distances. This individual requires visual supervision in the home and the community.

DESCRIPTION OF INDIVIDUAL: Individual is a 28-year-old male who lives in his family home with his parents and older brother. He is non-verbal and uses gestures and vocalizations to communicate his wants and needs. His family and his iPad are very important to him. It is important that any provider working with this individual is trained in crisis prevention and intervention techniques in order to respond to physical aggression. Individual can independently use his walker, but he requires assistance of someone guiding him to get up. It is important that others watch he doesn’t hit walls with his walker. When using a manual wheelchair for longer distances, he requires assistance from others to navigate at an appropriate speed and navigate uneven, rocky and/or icy surfaces.

PROVIDER REQUIREMENTS: Provider must be trained in crisis prevention and intervention techniques.

TRANSPORTATION NEEDS: Non-modified vehicle.

DIAGNOSES: Individual has the following diagnoses: Angelman’s Syndrome, tardive dyskinesia (TD-tongue is darting all over the place), constipation, seasonal allergies, PTSD and anxiety.

HOUSING: Family home in Perry, OH.

DAY PROGRAM/EMPLOYMENT: N/A
