DESCRIPTION OF REQUESTED SERVICES: This individual is looking for a residential provider for services in the home and workplace. She needs someone to come to her home, help her get to work, stay with her at work to help with tasks throughout the day, and take her home. She uses an electronic wheelchair. She would like a female staff. The individual would also require physical assistance with most ADL’s and to be aware of medical needs. The individual has her own wheelchair van and would prefer a staff that has a good driving record and the ability to drive the vehicle.

DESCRIPTION OF INDIVIDUAL: Individual is a 27 y/o verbal female who is employed in the community. She recently completed her master’s degree in social work. She is a bright and positive individual. She is a great writer and public speaker. She is a strong advocate for herself and others with disabilities.

PROVIDER REQUIREMENTS: Must be available on Monday’s starting 7/28/2025 from 7am – 5pm. Female staff only. Looking for someone who has decent typing skills and is comfortable with the computer (individual would be willing to train them if needed to set up her technology). They would also have to be comfortable with driving her accessible van to the west side of Cleveland (near the West Side Market). In addition, they would need to be comfortable with office work, including manipulating papers, typing in real-time as meetings progress, recording documentation as she dictates, and maintaining confidentiality. She will also need help using the restroom, eating lunch, and utilizing the elevator.

TRANSPORTATION NEEDS: modified vehicle; the individual has her own wheelchair van and would prefer a staff that has a good driving record and the ability to drive the vehicle

DIAGNOSES:  Cerebral Palsy, Spastic Quadriplegia, Complex Partial Seizure (History-has not shown seizure activity in many years), Cortical Blindness, Gastroesophageal Reflux, Scoliosis, Detrusor Sphincter Dyssynergia (Neurogenic Bladder), Alopecia Universalis 

HOUSING: lives in the Wickliffe area with her parents

DAY PROGRAM/EMPLOYMENT:  Employed in the Cleveland area
