DESCRIPTION of NEEDED SERVICES: Individual would like HPC services to help with his ADL’s specifically showering (needs support to transfer from wheelchair to shower chair, he is able to bear weight) up to 2x a week, total 4-6 hours weekly.

DESCRIPTION of INDIVIDUAL: Individual is a 33-year-old male who lives in Willoughby with his mom. He utilizes a specialized wheelchair and shower chair. He is able to provide support while transferring as he is weight bearing. Individual has a friendly small dog which he keeps in crate while providers are at his home.
PROVIDER REQUIREMENTS: The provider will deliver ISP services as determined at the initial Service meeting.
DIAGNOSES: CP, Hearing Impaired
SUPPORTS NEEDED: Individual can bear weight but needs assistance transferring from wheelchair onto shower chair.
