DESCRIPTION OF INDIVIDUAL: Individual is a 63-year male, looking to improve his employment skills so he can eventually gain competitive employment. The individual currently attends a voc hab program but would like a program where can work more hours each day. He currently is actively working for 1-2 hours per day. Individual does have explosive personality disorder. This typically manifests as yelling at peers or staff if he feels they have wronged him. This is his main barrier to competitive employment. The individual is redirectable by preferred staff when he exhibits these behaviors. His current program reports that he is one of their best workers and he takes pride in doing a good job.
PROVIDER REQUIREMENTS: Med cert 1 & 3. Insulin, Glucometer, epi pen, & inhaled chest medication.
TRANSPORTATION NEEDS: Non-Modified.
DIAGNOSES: Intermittent Explosive DO, Intellectual Disability, Anxiety, Diabetic, panic attacks, Hypertension, High Cholesterol, Tinnitus, Depression, COPD, Eczema, Periodontal disease, physical verbal maladaptive behavior D/O, Impulse control D/O, Angina, GERD, Esophageal anomalies, generalized anxiety D/O, constipation, mild Esophagitis, macular degeneration, ADHD, Hyperglycemia, Hyponatremia, Heel spurs, ulcer disease.
HOUSING: Individual lives in a home in Painesville with one other housemate
DAY PROGRAM/EMPLOYMENT: Attends voc hab 3x per week
