DESCRIPTION of INDIVIDUAL: This person is looking for a money management/authorized representative provider. She needs assistance with paying bills and recertifying for her benefits and housing. She needs assistance also managing a STABLE account.
PROVIDER REQUIREMENTS:
A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Anxiety/PTSD/Cerebral Palsy, Spasticity, Compound Hyperopic astigmatism, Onychomycosis, Seizure Disorder.
HOUSING: Lives in an apartment alone
DAY PROGRAM: Yes
