DESCRIPTION OF REQUESTED SERVICES: 64-year-old female individual looking for HPC/Transportation provider. Current HPC provider will be unable to support for 6 weeks starting 09/23/2026 (looking to fill this time to start). Client would like 1 day per week about 5 hours. Tuesday is a preferred day.

DESCRIPTION OF INDIVIDUAL: Individual is verbal and well-spoken. She is friendly. She has a lot of energy. Enjoys getting out and participating in community activities. She would like to go to the mall, shopping, and attend Lake County Board of DD planned events.

TRANSPORTATION NEEDS: Non-Modified vehicle
DIAGNOSES: Mild ID, High Blood Pressure, High Cholesterol, and Hypothyroidism, Factor 5
HOUSING: Lives in Kirtland, OH with family
DAY PROGRAM/EMPLOYMENT: Attends program 3 days a week in Mentor.
