DESCRIPTION OF REQUESTED SERVICES: Client is looking for a day program where she can start off going at least one day a week. As the client becomes more comfortable, they would consider attending more often. Once the client and family become comfortable, they would like transportation to be provided. Client would like to be involved with community outings and activities. Client would like the opportunity to meet new people
SEEKING AGENCY OR INDEPENDENT PROVIDER: Client is seeking an Adult Day Program where she can start off going one day a week and then maybe increase to more. At first client would not be interested in NMT but after she attends the program more would be interested in NMT.

DESCRIPTION OF INDIVIDUAL: Family is very important to the client. The client enjoys her dog and outdoor cats. Client attends church every Sunday. Client likes watching wrestling, kickboxing, and UFC. Routine is very important to the client and if there are changes, she is ok as long as she is provided enough notice. Client does not like to be rushed in any task or activity. Sense of style is important to the client. Client will pick her own clothes and will not change them if the weather changes. Client enjoys weather and keeping up with it. Client can be shy at first but once she is comfortable, she is able to communicate and ask someone for help if needed. Client needs physical assistance with dressing. Client is independent with toileting. Client does not require assistance with walking or getting up.
PROVIDER REQUIREMENTS: Client takes vitamins and medication for nose bleeds. Client does not need help with taking her medications.
TRANSPORTATION NEEDS: Non modified vehicle.
DIAGNOSES: Client is diagnosed with mild intellectual disability and cerebral palsy. Client is allergic to amoxicillin. Client is prone to get nose bleeds.
HOUSING: Client currently lives with her family in Perry.
DAY PROGRAM/EMPLOYMENT: Client is not currently interested in employment.
