DESCRIPTION OF REQUESTED SERVICES: In search of an ADS Program w/ NMT initially 2-3 days per week. Transportation to and from Program via Provider or another agency/independent provider.

DESCRIPTION OF INDIVIDUAL: A bright, fun, young lady not long out of school who previously looked forward to attending a daily program. Physical limitations including wheelchair for longer distances, walking with gait belt and assistance, and “drop” seizures requiring nasal spray emergency medication. Generally happy and easily redirected, she does enjoy sensory input and hearing things drop or “crash” so keep things out of reach that she may drop or throw or put into her mouth. She communicates primarily through gestures and facial expression and abrupt vocalizations. Understands English and Spanish.
PROVIDER REQUIREMENTS: Medication certification for rescue medication. NMT certification—can transfer to seat in a non-modified vehicle.
TRANSPORTATION NEEDS: Non-Modified Vehicle
DIAGNOSES: Syndrome Aicardi, Epilepsy, Autism, Cysts of eyelids, Constipation, Congenital malformations of corpus callosum, Abdominal swelling, Dorsopathy unspecified, Valgus deformity, Sterotyped movement disorders, Tremors unspecified, Global Developmental Delay. No known allergies.
HOUSING: Lives with her family in Painesville.
