DESCRIPTION of NEEDED SERVICES: This individual is a 27-year-old verbal female who is seeking a provider for Career Planning (job development) and Individual Employment Supports once the job is obtained.
FUNDING SOURCE: Level 1 Waiver

DESCRIPTION of INDIVIDUAL: She is interested in obtaining part-time employment. Some of her desired jobs would be working with animals, in a retail store, as a receptionist/cashier
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting. Prefers a provider who has experience working with individuals who have Prader-Willi so they can help navigate with the employer and work setting.
DIAGNOSES: Prader-Willi and Scoliosis.
HOUSING: Lives with family in Mentor
DAY PROGRAM: Attends a day program M-F but would adjust schedule based on work days
