DESCRIPTION of INDIVIDUAL: PROVIDER REQUIREMENTS: This person is a 34-year-old male with a good sense of humor. He resides by himself in Eastlake. He would prefer to work at home and requires a provider to help him find and maintain a job in the community.
A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Monosomy-18
HOUSING: Lives alone in single family home
