DESCRIPTION OF INDIVIDUAL: Individual is very social, likes to be busy, is in a wheelchair but has capabilities of using walker to transfer and using grab bars. May yell if upset, anxious or may throw things at others out of frustration or not getting attention right away. Does well in positive, encouraging and supportive environment. May refuse assistance from staff. Encouraged to use coping skills if having difficult time.
PROVIDER REQUIREMENTS: Provider to follow OISP services & supports.
TRANSPORTATION NEEDS: Modified vehicle – wheelchair
DIAGNOSES: Intellectual Disability, Cerebral Palsy, Spastic Quadriplegia, Anxiety & Scoliosis.
