DESCRIPTION OF REQUESTED SERVICES: Individual is seeking a provider for round trip non-medical transportation (NMT) to get to/from home and work. Home is located near intersection of Haines & Chapel Road, Madison and work is located near intersection of Heisley & Tyler Blvd, Mentor. He works M-F from 9:45am-6:15pm.

HOUSING: Resides with family in Madison, Ohio
DAY PROGRAM/EMPLOYMENT: Employed full-time in Mentor, Ohio – works Monday through Friday from 9:45am-6:15pm.
