DESCRIPTION of NEEDED SERVICES: Looking for an LPN or RN, to provide nursing services to an individual living in Kirtland, OH. The hours requested at 11PM-7AM.

DESCRIPTION of INDIVIDUAL:. Individual is a 29 year old male, living in Kirtland, Ohio. He has a funny and easy going attitude. Individual loves to cook and spend time with family. Individual has a strong sense of faith and is easy to get along with.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Individual’s primary diagnosis is arthrogryposis multiplex congenita (AMC). Individual’s other diagnosis’s include muscle disorder, abnormalities of breathing, heart failure, and scoliosis. Individual uses a power wheelchair for mobility, which they can manipulate themselves.
HOUSING: Individual lives with his mom in a house in Kirtland, OH.
DAY PROGRAM: N/A. Individual works full time from home.
SUPPORTS NEEDED: Individual is in need of a night nurse to ensure his medical needs are maintained and monitored throughout the night. Individual needs physical assistance with transfers, vital signs, personal care (bathing, dressing, shaving, oral care, peri-care), medication administration, and oxygen care.
