DESCRIPTION OF REQUESTED SERVICES: Individual is a male looking for a male DSP to go out in the community with him 3-4x a week. He is available Mon-Wed in the afternoon/evening and Saturday afternoon/evening.
SEEKING AGENCY OR INDEPENDENT PROVIDER: Either – but would want consistent staff member and wants a male.

DESCRIPTION OF INDIVIDUAL: Individual is a 49-year-old male who enjoys companionship and getting out into the community. He would like a provider to take him out to places like restaurants, the mall, parks, Deepwood Activities, Deepwood Dances.
PROVIDER REQUIREMENTS: Would like male staff.
TRANSPORTATION NEEDS: Needs transport in non-modified vehicle.
DIAGNOSES: Mild intellectual disability, Tourette’s disorder, Anxiety, OCD, acid/esophageal reflux
HOUSING: Lives in Willowick and would want to receive services in the community. Pick up and drop off at home in Willowick.
DAY PROGRAM/EMPLOYMENT: Works in the community with IES supports.
