DESCRIPTION of NEEDED SERVICES: This is a request for a 23-year old male living in Kirtland seeking HPC services within his home and out in the community. He is seeking between 9-12 hours of services throughout the week about twice a month. He enjoys music, writing, video games, sports, and drawing.

DESCRIPTION of INDIVIDUAL: This individual struggles with anxiety and likes to know what is going on in his life in advance. He is flexible with change as long as he is notified in advance of any change. Staff should have knowledge and experience understanding Autism and anxiety disorders to be able to keep him from becoming anxious about his schedule. This individual enjoys learning new facts about history, spirituality, and war history.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Autism, anxiety, ADHD, and OCD
HOUSING: Individual lives in Kirtland with his mom and grandparents.
DAY PROGRAM: Individual does not attend a day program.
SUPPORTS NEEDED: This Individual is seeking assistance with socialization, and transportation out in the community to access outings of his choice. He does need help with understanding money management and learning how to budget his funds in his possession. This individual does struggle with anxiety and would require staff to be flexible and understanding his anxiety with schedule change. He prefers to have advanced notice of any change in schedule or routine for his services.
