DESCRIPTION OF INDIVIDUAL: Individual lives in Mentor, Ohio. Individual requires assistance with all ADL’s and IADL’s. Individual utilizes a ventilator for oxygen, g-tube for medication and nutrition. These shifts would involve taking the individual out in the community.
PROVIDER REQUIREMENTS: Provider would have to be an RN or LPN.
TRANSPORTATION NEEDS: Modified vehicle, which the family has.
DIAGNOSES: Muscle disorder
HOUSING: Mentor, Ohio.
DAY PROGRAM/EMPLOYMENT: N/A
