DESCRIPTION of INDIVIDUAL: Client is 65 year old female who has Mild Intellectual Disability. She enjoys socializing and doing arts and crafts, as well as going out into the community. She is fully ambulatory, however she does use a walker to assist. She needs physical assistance with toileting and showering, and total assistance with laundry.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Mild Intellectual Disability, Diabetes
HOUSING: She lives in Mentor, Ohio by herself.
SUPPORTS NEEDED: HPC in her home
