DESCRIPTION of NEEDED SERVICES: 24 year old male individual seeking Adult Day Services and Non-Medical Transportation services.

DESCRIPTION of INDIVIDUAL:.This individual speaks in simple words and phrases rather than full sentences. He responds best to clear/concise directions. He needs verbal prompting to initiate conversations. He really enjoys watching Blue’s Clues, playing on his tablet, singing songs, and going to McDonalds. He does not like loud large settings. He has anxieties with unfamiliar people. Overall he is very easy going. He is looking for a day program setting where he can participate in fun activities. He has no noted behaviors. He utilized the ICF setting over the last year after his mother had passed and is transitioning to a waiver setting once a home is secured.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Autism and Epilepsy.
HOUSING: Currently at a Cuyahoga ICF, looking to relocate to a waiver setting. Housing search continues, but guardian would like to also secure a day program.
DAY PROGRAM: N/A at this time.
SUPPORTS NEEDED: Provider would be responsible to ensure that this individuals ADL’s are maintained and he is supported to learn new skills. He needs verbal to physical assistance with all ADL’s to include daily cleanup of his environment and hygiene needs. He needs supports to access community locations and needs to be within arm’s length when navigating high traffic areas/parking lots. He has limited pedestrian safety skills. He thrives with a written calendar so that he knows what to expect and when. He will need total support with management of finances.
