DESCRIPTION of NEEDED SERVICES: Individual is in need of Residential Respite Certification (52 hrs monthly), provided Monday-Friday 2:00pm-5:00pm flexibility with days and hours. Providing communication and general supervision in the home or community-past history of seizures, assist with meal prep and minimal hygiene tasks.
SEEKING AGENCY OR INDPENDENT PROVIDER: Agency or Independent Residential Respite Certified Provider (father is Shared Living provider).

DESCRIPTION of INDIVIDUAL: Friendly, good natured and social 37-year-old male, limited communication skills, past history of seizures.
PROVIDE REQUIREMENTS: This provider will deliver Residential Respite Services, as father is Shared Living provider, ability to provide 52 hours a month (13 hrs weekly) and ISP services as documented in the ISP, to a kind, friendly, mild-mannered 35-year-old male.
TRANSPORTATION REQUIREMENTS: Individual is mobile and can transport in a non-modified vehicle.
DIAGNOSES: Autism, moderate ID, epilepsy, depression, anxiety.
Housing: Individual lives with his father in their home in Mentor.
