DESCRIPTION of NEEDED SERVICES: This individual is a 79-year-old male who lives in Perry, Ohio with three roommates. He is looking for a vocational habilitation program to attend with non-medical transportation part time to learn work skills and be busy during the day. He would be interested in attending 3-4 days per week.

DESCRIPTION of INDIVIDUAL: PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Seizure disorder, Severe Intellectual Disability, Hepatitis B carrier, Vitamin Deficiency, Epigastric Hernia, OCD, Cataract (mild) left eye, Anemia, bilateral glaucoma (more advanced right eye), bilateral nuclear cataracts.
HOUSING: He lives in a group home in Perry.
DAY PROGRAM: He just recently stopped going to a vocational program.
SUPPORTS NEEDED: He is a very good worker and enjoys being busy. He does have OCD which may result in him stopping his task to fix whatever he sees is out of place. He needs visual supervision to ensure that he does not elope to pick up a stray piece of trash. He has a seizure disorder but has not had one in 15 years.
