DESCRIPTION of NEEDED SERVICES: This individual is an 20 year old female who resides in Madison. She is looking for an HPC provider, approximately 10 hours per week that will provide opportunities for socialization and community access. She would require transportation in a non-modified vehicle.

DESCRIPTION of INDIVIDUAL: This individual enjoys a variety of activities including drawing, watching videos on YouTube, going on swings, swimming, and going to the store.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Autism spectrum disorder, ADHD, intellectual disability, bipolar disorder
HOUSING: She lives in Madison with her mother and grandmother.
DAY PROGRAM: She does not attend a day program at this time.
SUPPORTS NEEDED: She would require assistance navigating curbs and steps, as well as assistance to get in and out of a van/vehicle.
