DESCRIPTION of NEEDED SERVICES: Individual needs Non-Medical Transportation 5 days per week on Monday through Friday. Arrive by 9am and pick up at 2pm. Home and Day Program are both in Mentor.

DESCRIPTION of INDIVIDUAL: A 41-year-old male with a great personality and smile but has difficulty with effective communication, which can cause occasional agitation. He has a communication device to assist with communication but needs to be reminded to use it. He worked in the community without support until recently and now attends ViaQuest 5 days per week. He’s very pleasant and likeable but would do better in a vehicle with few or no other passengers as he likes attention from non-peers and can be temperamental when he doesn’t get his way (demanding the front seat, to control the radio, etc). Likes watching WWE, listening to music and dancing, and activity in the community.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting. Few or no passengers during transportation.
DIAGNOSES: Down Syndrome, high cholesterol and acid reflux. Allergies: Augmentin (severe), dust, cats, pollen, smoke.
HOUSING: Lives in congregate setting in North Mentor.
DAY PROGRAM: Monday-Friday at ViaQuest on Shady Ln.
SUPPORTS NEEDED: Transportation to and from ViaQuest.
