DESCRIPTION of NEEDED SERVICES: Looking for and ADS program for a male in his late 20’s on the autism spectrum.

DESCRIPTION of INDIVIDUAL:. Individual is a male in his late twenties. Individual communicates through non-verbal communication. Individual may use a communication board or lists. Individual enjoys relaxing, watching movies (Disney), and making lists. Individual strives off of routine.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Individual’s diagnoses include autism, anxiety, speech/language disorder, and diabetes. Individual is currently in the process of being diagnosed for sleep apnea, so has been falling asleep frequently during the day. In the past, individual has exhibited
HOUSING: Individual lives at home with his parents in Painesville, Ohio.
DAY PROGRAM: Individual is currently looking for a day program to attend.
SUPPORTS NEEDED: Individual needs assistance with setting up and following daily routine. Individual may needs verbal prompts to use the bathroom. Individual needs assistance making purchases, in emergency situations, and heating up food.
