DESCRIPTION of NEEDED SERVICES: 39-year-old female looking for HPC supports/transportation. Seeking up to 5 hours weekly. Specifically needs evening supports to go to dance practice, planned Aktion club meetings at Lake County Board of DD, other planned events/activities. Assistance to go clothes shopping, etc. Upcoming things needing support August 6, August 13th, August 22nd (can discuss further with anyone who responds). Her current HPC provider is not available in August & September.

DESCRIPTION of INDIVIDUAL:. This individual is verbal. She is able to speak clearly and directly and be understood by those familiar and unfamiliar with her. She loves traveling, taking photos and going to craft stores. She is involved in Special Olympics, loves to dance & enjoys seeing her friends and family. She is independent with most tasks with some verbal prompting. She is kind and helpful.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Autism, Mild ID
HOUSING: Currently living with her aunt in Willowick
DAY PROGRAM: Voc Hab program M,W,F in Willowick
SUPPORTS NEEDED: Client will need assistance to get to and from all planned activities. She will need assistance at times to make purchases/determine her wants vs needs. She needs things explained clearly to her and needs some processing time.
