DESCRIPTION OF REQUESTED SERVICES: Looking for an HPC provider to provide HPC services to male in his thirties. The hours needed are Saturdays 11AM-3PM.

DESCRIPTION OF INDIVIDUAL: Individual enjoys going to water parks, amusement parks, museums, playing video games, dinosaurs, and writing his thoughts on movies or documentaries. Individual likes routine and advance notice of change in routine.
PROVIDER REQUIREMENTS: HPC and HPC Transportation. Provider needs to be comfortable with dogs and a non-smoker. The individual would prefer a female provider over the age of 40.
TRANSPORTATION NEEDS: Non-Modified
DIAGNOSES: Autism and ADHD
HOUSING: Individual lives in Mentor, Ohio.
DAY PROGRAM/EMPLOYMENT: Individual is employed in the community.
