DESCRIPTION of INDIVIDUAL: This individual is a four-year old female who lives at home with her parents in Madison. She likes Peppa Pig, listening to music, singing and helping others. She requests assistance by reaching out for someone near by.
PROVIDER REQUIREMENTS: A provider would need to be able to safely transport this individual to and from her day program on required days.
DIAGNOSES: Triple X Syndrome, Autism, Mixed Receptive/Expressive Communication D/O
HOUSING: Lives at home with parents in Madison.
DAY PROGRAM:. She attends LLA Therapy in Mentor from 9:00 AM – 3:30 PM on a schedule of alternating weeks: Wednesday/Thursday, then Monday/Tuesday/Friday
SUPPORTS NEEDED: This individual need to be picked up in time to be at her day program by the starting time of 9:00 AM and then picked up to be transported home at 3:30 PM. She will need door to door service and assistance with getting in and out of the vehicle safely and assistance with her seat belt. Her school system denies responsibility to transport her, and her parents are unable to do so.
