DESCRIPTION OF REQUESTED SERVICES: Individual is a 28-year -old female seeking a provider for round trip non-medical transportation (NMT) to get to/from home and her group employment site. Home is located near the intersection of Lakeshore Blvd and Heisley Road in Mentor and the group employment site is located off Science Park Drive in Beachwood. The trip is approximately 25 miles one-way. NMT is needed Monday – Friday, drop off in Beachwood by 9:00am and pick-up at 3:00pm.

DIAGNOSES: Prader Willi Syndrome, Scoliosis
HOUSING: Resides with family in Mentor, Ohio
DAY PROGRAM/EMPLOYMENT: Will be starting at a group employment site around August 18, 2025, and attended Monday-Friday.
