DESCRIPTION of INDIVIDUAL: Individual is a 61-year-old male. He loves Cleveland Sports and spending time with his family. He needs encouragement to make good choices and support for all his daily living skills.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Moderate ID, Down Syndrome, Hypothyroidism, Vitamin D Deficiency, High Cholesterol, Lactose Intolerant, Depression, and Arthritis.
HOUSING: Individual lives in a congregate setting in Painesville.
DAY PROGRAM: Individual attends ADS program, 5 days a week, M-F.
SUPPORTS NEEDED: Money management provider and payee. Assistance with budgeting, bill payment, maintaining benefits for Social Security, SNAP, and Medicaid, managing accounts. He likes to receive his spending money on a monthly basis.
