DESCRIPTION of INDIVIDUAL: He is a kind individual who has a sense of humor and enjoys joking around with others. He is mechanically inclined and likes to learn complex tasks. Recently, he has been working through some medical issues. He becomes nervous when given too much instruction at once and may choose not to do something. He has his driver’s license and really would like to save up for a car. He likes to please others so he may say what he thinks you want to hear. He benefits from reminders that he is not disappointing you and needs encouragement to share his thoughts and goals.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES:Epilepsy, Depression, ADHD, Hypertension, PTSD, Hypothyroidism, Hyperglycemia, Hyperlipidemia, Herniated Disc, Vitamin D Deficiency, Low testosterone, Recent Hx Colon Cancer, Constipation, Gout, Anxiety.
HOUSING: Living in a waiver home with 2 non-related housemates.
DAY PROGRAM: Looking for a VocHab program.
SUPPORTS NEEDED: Supports will be needed to manage any funds if used during his time at a VocHab program. He will need assistance to administer medications as prescribed. He needs assistance to stay on task. He will benefit from a job that has the opportunity to sit and do tasks as he has medical issues. He will need support to prepare his things for lunch. He has hopes to gain community employment and will need supports to learn community employment options/job skills.
