DESCRIPTION OF REQUESTED SERVICES: This individual is a 20-year-old male who resides in Wickliffe OH. The individual’s family is seeking an NMT driver to pick him up at his ADS in Willoughby (~2pm) and take him to his home in Wickliffe (~2 miles) M-F. At this time, they are seeking afternoon transportation only.

DESCRIPTION OF INDIVIDUAL: This Individual communicates with single words or 2–5-word phrases. He is very expressive and uses facial expressions and gestures to convey his emotions. Individual is independent with mobility and is very active. He does not appear to have an adequate understanding of safety skills or cause and effect. Individual has been known to make verbal or gestural threats and has a history of elopement. Individual may exhibit SIB or physical aggression. He benefits from having extra time to process when you speak with him, also by minimizing distractions in his environment and speaking in a positive tone of voice. This individual loves basketball, football, music, cars, visiting parks, community outings, board games, and live music.
PROVIDER REQUIREMENTS: Training in Crisis Prevention and Intervention techniques is preferred but not required. Certified in NMT or HPC transportation.
TRANSPORTATION NEEDS: Non modified vehicle.
DIAGNOSES: Autism, Impulse Control Disorder, ADHD, Tourette’s and Aggressive Behavior.
HOUSING: Wickliffe.
