DESCRIPTION of NEEDED SERVICES: Individual is looking for a placement in a supported living home.

DESCRIPTION of INDIVIDUAL:. Individual is a 26 year old male.  Although he is verbal, he rarely talks at his day program, and when he does, it is very quiet.  He has difficulty communicating his emotions and can sometimes engage in self-injurious behavior by banging his head.  He enjoys going places and doing new things, especially outdoor activities.  He loves going to Round 1.

PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.  Staff will need to be certified to provide oral/topical medication administration.

DIAGNOSES: Autism, Anxiety, MTHFR mutation

HOUSING: Individual currently lives with his mother in the family home in Mentor.  Due to medical concerns with his mom, he is looking for residential placement in a supported living home.

DAY PROGRAM: Individual currently attends a day program in the Eastern mentor Area 2-3x/week.  However, his days of attendance can be increased.

SUPPORTS NEEDED: Individual is independent with ADLs, although he may need verbal reminders.  He requires physical assistance for nail care and laundry.  He is able to use a microwave and toaster oven to heat/toast things, as well as the stovetop to heat up prepared/simple foods.  He requires physical assistance to plan and prepare most foods.  He needs total assistance to coordinate medical and total assistance for all financial matters.  Staff will need to be certified to provide medication administration.
