DESCRIPTION OF INDIVIDUAL: This individual is female. Enjoys spending time with her friends, going to program, outings, singing, etc. She is able to text/call her mom. She spends her time alone when she’s home from program until her mom arrives.
PROVIDER REQUIREMENTS: This individual would need transportation to and from home for program M-F.
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: Autism Spectrum Disorder.
HOUSING: She lives in Mentor near the Mall.
DAY PROGRAM/EMPLOYMENT: Euclid/ Help Foundation Inc.
