DESCRIPTION of NEEDED SERVICES: This individual is a 19-year-old verbal male who is seeking a provider for Individual Employment Supports. He will be starting a new job in Painesville, Ohio in August, 2024 and is seeking a job coach to assist him with learning his job. He will be working at a school as a floater (assists in various areas/positions as needed throughout the school).

DESCRIPTION of INDIVIDUAL: Individual is currently employed part-time and working independently. He was hired at a school as a floater and will be starting this position in August, 2024. He would like to have an Individual Employment Support provider to assist him with learning his new job.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
