DESCRIPTION of NEEDED SERVICES: HPC services with some HPC transportation to community outings. Supervision: Up to 8 hours alone time in the home and community with added time following check-in with staff. General supervision in the home and in the community.

DESCRIPTION of INDIVIDUAL: Individual is a 40 year old male who is verbal, speaks English, and can communicate his wants, needs, and feelings. Individual enjoys participating in community outings and activities, Special Olympics, and video games. He is independently mobile and needs no assistive devices. Individual also drives his own car.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Mild MR, Asperger’s, Major Depressive Disorder recurrent, ADHD, Keratoconus, Sleep Apnea, Obesity
HOUSING: Residential home in Madison with three male housemates.
DAY PROGRAM: Community Employment
SUPPORTS NEEDED: Verbal assistance with personal hygiene tasks. Physical assistance with household chores, laundry, meal planning, meal preparation, and shopping for groceries. Individual has no healthcare concerns needing continuous monitoring or attention, however, does need assistance with all aspects of healthcare appointments.
