DESCRIPTION of NEEDED SERVICES: HPC services and HPC transportation services with no alone time and OSOC during sleep hours. General supervision in the home and visual in the community.

DESCRIPTION of INDIVIDUAL: Individual is a 60-year old male who enjoys watching television, coloring, and puzzles. He is verbal, speaks English, and can communicate his wants, needs, and feelings. Noisy, stressful, or crowded environments can cause him to shut down, and he is supported best with praise, encouragement, and verbal reinforcement.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Moderate ID, Partial right side paralysis, periodontal disease (severe gum disease), Intermittent Explosive Disorder, High Cholesterol, Depressive Disorder NOS, Hypothyroidism
HOUSING: Residential home in Madison with three male housemates.
DAY PROGRAM: N/A
SUPPORTS NEEDED: Verbal to physical assistance with personal hygiene tasks, household chores, laundry, meal planning, meal preparation, and shopping for groceries and personal items. Medication administration and help finding and participating in community activities. HPC transportation. Individual has no healthcare concerns needing continuous monitoring or attention.
