DESCRIPTION of NEEDED SERVICES: Individual is in need of NMT transportation to/from Group Employment in Beachwood Ohio and home in Willoughby. Approximately 20miles each way/23 minutes. Scheduled would be Monday-Friday 9-1 for 2 weeks and then Monday-Friday 9-3 going forward. Individual is interested in moving to Old Brooklyn in Cuyahoga in the future but currently resides in Willoughby.
FUNDING SOURCE: Level 1 Waiver
DESCRIPTION of INDIVIDUAL: 20 year old young lady is friendly, verbal and ambulatory, she does not require accommodations.

HOUSING: She currently resides in Willoughby, Ohio. Intends to move to the Old Brooklyn area of Cuyahoga County in the future.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined within the ISP.
DIAGNOSES: Mild Cognitive Impairment, ADHD
DAY PROGRAM: Group Employment with UCP.
SUPPORTS NEEDED: Non-Medical Transportation
