DESCRIPTION OF REQUESTED SERVICES: 13-year -old male seeking homemaker personal care supports within his home to provide general supports, and access to community outings and activities. Individual is seeking supports from 2-6pm 3 days per week, and 5 hours every other weekend.

FUNDING SOURCE: Level 1 waiver

DESCRIPTION OF INDIVIDUAL: Individual does have a history of physical aggression to include punching and kicking towards peers or while in a motor vehicle. He works best with a consistent routine, simple and direct language, and having a quiet and calm space to go to when he is upset.
PROVIDER REQUIREMENTS: Preferred provider would have a form of crisis intervention curriculum training but is not formally required.
TRANSPORTATION NEEDS: Individual requires a non-modified vehicle.
DIAGNOSES: Individual is diagnosed with Autism, Oppositional defiance disorder, receptive language disorder, and verbal apraxia. He has no known allergies.
HOUSING: Individual resides with his mother and sister in his home and is seeking in home and community-based activities for supports.
DAY PROGRAM/EMPLOYMENT: Individual attends School Monday-Friday.
