DESCRIPTION OF REQUESTED SERVICES: Individual is interested in HPC services in or out of the home when her day program is closed or is home sick as well respite time for mother on a regular basis. Schedule would be developed with the family. Individual is able to walk on her own but sometimes needs assistance (a hand) when walking down steps or on uneven surfaces. Individual will need total assistance in meal prep and at times will need physical assistance using the restroom. Individual needs auditory supervision in the home with frequent visual check ins and visual supervision in the community. Individual enjoys going out into the community.

DESCRIPTION OF INDIVIDUAL: Individual is nonverbal but is able to communicate with gestures to people that know her. Individual loves Veggie Tale books and movies. Her Veggie Tale books are very important to her. Individual likes to be read to. Individual needs time to process her thoughts and to be given choices. Individual participates in Miracle League and Metropark Adaptive Sports. Individual likes to be around people and watch people. She can be hesitant at first to join in activities, but with time with participate with assistance from her support person.
PROVIDER REQUIREMENTS: Family would prefer a female provider who is non-smoking. They would like someone who is energetic and able to be active with individual.
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: Individual is diagnosed with Down’s Syndrome, Hypothyroidism, OCD, Tourette’s, ASD, and ADHD.
HOUSING: Individual lives with her mom in Perry but will visit her dad out of state several times a year.
DAY PROGRAM/EMPLOYMENT: Attends a day program in Mentor.
