DESCRIPTION OF REQUESTED SERVICES: 45-year-old male in search of M-F ADS program. The site must be wheelchair accessible as the client utilizes a motorized wheelchair.

DESCRIPTION OF INDIVIDUAL: Individual is always in good spirits! He is nonverbal and depends on staff to be proactive about his needs. He requires support for all transfers. He uses a gait trainer daily to maintain his strength. He has G-tube feeds, but will not require this during programming hours, instead he has “pleasure feeds”, which his home provider will send with him. Will need staff to assist in changing his adult briefs and monitor this need frequently throughout the day. He loves R&B music, particularly Janet Jackson & Whitney Houston. He enjoys being a part of the group and being offered all opportunities to spend time in the community.
PROVIDER REQUIREMENTS: Follow all services as outlined in the ISP.
TRANSPORTATION NEEDS: Modified vehicle is required.
DIAGNOSES: Profound Intellectual Disability, Cerebral Palsy, Dysphagia, Oroboccofacial Dyskinesia, Contractures (All Extremities), CP Spasticity, Hypothyroidism, Anxiety Disorder, Depression, G-Tube for nutrition.
HOUSING: Currently resides in a waiver home in Madison
DAY PROGRAM/EMPLOYMENT: In search of new program M-F. He prefers not to work.
