DESCRIPTION of NEEDED SERVICES: 71-year-old female individual and 75-year-old male individual that are married and live together. They are looking for an HPC provider to assist with activities of daily living and accessing the community. Schedule is not yet determined and can be flexible.

DESCRIPTION of INDIVIDUAL: Both individuals are verbal and live alone with drop in HPC services. Individuals have been married for over 30 years. They like to go shopping and out to eat. They enjoy watching TV and relaxing at home with their pet bird.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting. Provider needs reliable transportation.
DIAGNOSES: Male individual – Mild ID, High Cholesterol, Heart Murmur, gastritis, arthritis, overactive bladder. Female individual – Mild ID, sensory loss in right foot, depression, right leg shorter, GERD, IBS, Cephalgia (headaches), High Blood Pressure, incontinence, Sleep Apnea, High Cholesterol; Hx breast cancer Stage 1 right side
HOUSING: Live in a house in Mentor-on-the-Lake, OH.
DAY PROGRAM: Both attend a day program two days a week.
SUPPORTS NEEDED: Individuals need assistance with scheduling, attending, and transportation to and from their doctor’s appointments. They need assistance budgeting their personal spending money. Need physical assistance with meal planning and grocery shopping and verbal to physical prompts to clean their house. Need assistance with accessing activities in the community they enjoy (going out to eat, going shopping, and going to local events).
