DESCRIPTION OF REQUESTED SERVICES: 36-year-old female individual in search of a provider available on average 8 hours per month for HPC supports while family is away.

DESCRIPTION OF INDIVIDUAL: Client lives in Concord Twp., with her mother and father. She is very social, she is mobile, and she has limited verbal communication skills. She requires supervision for health and safety. Individual struggles with anxiety while transitioning or experiencing change. She has a history of physical aggression towards staff, or self-injurious actions towards herself during these transitions when not provided with advanced notice of change. Individual uses a visual calendar and social stories to help transition her to various activities in the community. Communication support, Supervision, Support with Diet, Assistance with Transitions, Money Management of Program Funds, Assistance with Toileting, Dressing, Assistance with Meal Preparation, Transportation, etc.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non-modified vehicle
DIAGNOSES: Autism, hypotonic Cerebral palsy, seizure disorder, and anxiety
HOUSING: Concord twp home with parents.
DAY PROGRAM/EMPLOYMENT: Attends Day program 3 days per week
