DESCRIPTION of INDIVIDUAL: PROVIDER REQUIREMENTS: This person is a friendly 22-year-old male with a good sense of humor. He resides with his parents in Mentor and works at Breckenridge Rehabilitation. Individual has difficulty with social cues and recognizing non-verbal communication of others. Individual requires a consistent schedule with one specific staff. A male staff is required. This young man needs a mentor, someone with patients and follow through. Provider would need to provide IES services in accordance with the ISP. Individual may need assistance regulating emotions due to the behavior or communication of co-workers. A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
