DESCRIPTION OF INDIVIDUAL: Individual seeking a day program that can provide a variety of activities/outings. Individual does not like loud/crowded areas. She enjoys puzzles, helping others, organizing items and watching others participate in activities. She is non-speaking and relies on those familiar with her to help communicate her needs. She communicates through vocalizations/stimming/jumping. She may throw/break items when she is upset.
PROVIDER REQUIREMENTS: Provider will not need to pass meds at program
TRANSPORTATION NEEDS: Non-modified vehicle
DIAGNOSES: Severe IDD
HOUSING: Individual lives in Mentor near SR 615
DAY PROGRAM/EMPLOYMENT: Not currently attending a program
