DESCRIPTION OF REQUESTED SERVICES: Individual is looking for HPC provider for 1:1 support 7 days/week other than when he is at his day program. Individual also requires HPC support throughout the night to ensure he is using his Bi-PAP machine.

DESCRIPTION OF INDIVIDUAL: Individual is a 38-year-old male, who lives alone in his apartment in Wickliffe, OH. He requires physical to total assistance with all ADL’s, meal planning and preparation, transportation and engagement in community activities. This individual does not like dogs, loud environments or places with high ceilings. He enjoys watching tv/movies, listening to music and going to restaurants. Due to PTSD and history of trauma and abuse, this individual has a history of engaging in aggression, property destruction and name calling, but this has drastically reduced and typically only arises in instances of extremely high anxiety. This individual requires visual supervision in the community, audible supervision in the home and has no alone time for safety reasons.
PROVIDER REQUIREMENTS: Medication Certification 1.
TRANSPORTATION NEEDS: Non-modified
DIAGNOSES: Down’s Syndrome, Hypothyroidism, Vitamin D deficiency, Anxiety, PTSD, gastroesophageal reflux disease, rosacea, severe sleep apnea, low iron.
HOUSING: Wickliffe, OH.
