DESCRIPTION OF REQUESTED SERVICES: Individual is a 39-year-old female seeking a job coach to assist with communications and concerns at her place of employment, Giant Eagle in Mentor-on-the-Lake. Work schedule: Sunday, Monday, Tuesday: 12:00 pm – 4:00 pm. She has been successfully employed since spring 2026. The individual is recovering from surgery and is expected to return to work in October.

DESCRIPTION OF INDIVIDUAL: The individual is verbal, speaks English, and is fully independent with mobility. She is very social, enjoys talking with others, and is motivated by frequent activities and community engagement. Although she has had no recent incidents, during work and personal time, she has a history of difficulty regulating her emotions at times. When frustrated or overwhelmed, she may have emotional outbursts, including the use of foul or threatening language. During these moments, she benefits from reassurance, patience, and calm redirection. Staff must use a calm, friendly tone, ensure she feels heard, and offer choices to help her maintain a sense of control. She may need reminders about personal boundaries and appropriate relationship interactions. She responds best to staff who demonstrate patience, provide clear expectations, and support her in managing her feelings in a respectful and person‑centered manner.
PROVIDER REQUIREMENTS: This individual benefits from job‑coaching support to help her communicate effectively with supervisors, understand workplace expectations, and resolve concerns that arise during her shifts and deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Currently seeking NMT, non-modified
DIAGNOSES: Mood Disorder, NOS, Mild Intellectual Disability, Fetal Alcohol Syndrome, Acid reflux, Migraines, Growths on Liver; Rheumatoid Arthritis and Gout.
HOUSING: Individual resides in an apartment in Mentor-on-the-Lake.
DAY PROGRAM/EMPLOYMENT: Individual is not currently employed or attending a day program.
