DESCRIPTION OF INDIVIDUAL: Individual is 35 years old and relies on a wheelchair for general mobility but does practice walking with physical assistance for very short distances to maintain muscle strength and flexibility. Individual has become an increased fall risk when assisted by caregivers and is seeking a tram type of walker to help him continue his exercise with adequate support. He resides in the family home with his parents in Wickliffe.
PROVIDER REQUIREMENTS: Certified to provide the equipment requested. Physical therapy order and evaluation are completed.
