DESCRIPTION OF INDIVIDUAL: This individual communicates verbally but is echolalic and will typically repeat back the last word said to him. He uses gestures like pointing to what he needs or wants. He is easy going and likes it when others laugh. He needs many verbal prompts to stay focused on a task and engaged. He needs to be aware of the schedule/expectations ahead of time, so he feels more comfortable with his day. He needs choices throughout his day and does best when he is busy. He enjoys community outing like bowling, going to the movies, etc. He often seeks attention from others whether this is negative or positive. He often takes other clients’ food/drinks or will push over tables or garbage cans (in a non-aggressive way). He values his family, loves to feel part of the group, attend camp yearly, and participates in many activities with his mom’s support.
PROVIDER REQUIREMENTS: Staff must be able to implement all aspects of the OISP. Individual is a C acuity and needs visual supervision at all times.
TRANSPORTATION NEEDS: Non-Modified
DIAGNOSES: Down Syndrome, Autism, Anxiety, Alopecia
HOUSING: Client resides in Madison with his mother who is his Shared Living Provider.
DAY PROGRAM/EMPLOYMENT: Looking for new program M-F
