DESCRIPTION OF REQUESTED SERVICES: :45-year-old male who lives in Mentor with two housemates, receives up to 22 hours/day supervision and attends an ADS program 5 days/week. He would like to explore a new ADS program. He enjoys going out in the community and volunteering. He enjoys being active and social with others.

DESCRIPTION OF INDIVIDUAL: This individual has cerebral palsy, uses a wheelchair, and requires staff assistance for all ADLs. He would need assistance with transportation, some transfers, toileting, and supervision/assistance with purchases in the community. He is independent in most other tasks at his ADS, fully verbal, and generally able to express his needs.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Individual would need a modified vehicle for transportation.

HOUSING: Currently lives in Mentor with 2 housemates
DAY PROGRAM/EMPLOYMENT: Exploring a new day program.
