DESCRIPTION OF REQUESTED SERVICES: Individual is looking for a waiver home with an established HPC provider that can assist with ADL’s. Environment must be accessible by wheelchairs. Would prefer Painesville area, but open to Lake County. Individual needs physical assistance with navigating curbs/loading/unloading from a vehicle, scheduling and attending all medical appointments, total assistance with cleaning/chores, physical assistance with administering medication, physical assistance with showering, oral hygiene, hair care, shaving, nail care, physical to total assistance with meal prep/planning and grocery shopping, and physical assistance with accessing the restroom and wiping after a BM.

DESCRIPTION OF INDIVIDUAL: 32-year-old young man who uses an electronic wheelchair to get around. Requires wheelchair accessible transportation and environment. He is fun, easy going and kind. Adapts easily to change and is very social. Likes to hang out with friends and talk, use social media, play Xbox, watch WWE wrestling and spend time with his family.
PROVIDER REQUIREMENTS: Home must be wheelchair accessible and provide wheelchair accessible transportation. Requires wheelchair accessible building, restrooms and doorways.
TRANSPORTATION NEEDS: modified vehicle.
DIAGNOSES: developmental delay, progressive neuromuscular/neuropathy disease, epilepsy, and seizures
HOUSING: Resides in a home in Painesville, OH
DAY PROGRAM/EMPLOYMENT: Attends 3x/wk in Painesville
