DESCRIPTION OF INDIVIDUAL: This individual is a 12-year-old female who resides in Willoughby Hills. She has a primary diagnosis of cerebral palsy and a history of seizures. Individual uses a power wheelchair for mobility and communicates verbally. She enjoys watching TV, hanging out with her siblings, shopping, and listening to music. Individual would like to be taken out into the community if possible.
PROVIDER REQUIREMENTS: Provider needs to be able to manually lift and assist with transfers.
TRANSPORTATION NEEDS: Modified vehicle
